Gum hygiene and the first teeth coming through

Even before the first tooth appears, the gums should be cleaned with a damp gauze pad. When the first baby tooth comes through, at around 6 months, it's the ideal time for the first visit. We check how the teeth are coming through, guide parents on hygiene habits and pick up on any early issues.

First formal visit, preventive sealants and habit monitoring

The full set of baby teeth is usually in place by around age 3. At this stage we carry out six-monthly check-ups, apply sealants to the baby molars and give professional fluoride treatment. We also assess habits such as dummy use, bottle feeding and thumb-sucking, which can affect jaw development.

Mixed dentition, sealants for permanent teeth and interceptive orthodontics

Tooth replacement begins. The first permanent molars come through at around age 6 and are the most important: we seal them straightaway. We assess the alignment of the new teeth and, if we spot spacing or bite problems, we start interceptive orthodontics to make the most of the child's growth and avoid longer treatment later on.

The foundation of everything. A visit every 6 months lets us catch early decay before it spreads, monitor tooth development and reinforce hygiene habits at home. Professional cleaning removes the tartar a toothbrush can't reach.

Fissure sealants

Molars have deep grooves where bacteria build up even when a child brushes well. We apply a sealant resin that covers those grooves and protects the tooth for years. It's the most effective preventive treatment there is. Painless, quick and long-lasting.

Professional fluoride treatment

Fluoride strengthens the enamel and makes teeth more resistant to the acids produced by bacteria. Professional application at the clinic uses higher concentrations than toothpaste at home, with results that last for months. We recommend it twice a year for children at higher risk of decay.

Interceptive orthodontics

When we spot spacing problems, crossbite or developmental issues between ages 6 and 10, we act before the bones finish growing. Orthopaedic appliances at this stage guide jaw development and can avoid extractions or much longer treatment later on.

Many people think baby teeth don't matter because they're going to fall out anyway. It's the most common mistake in children's dental health. Baby teeth serve critical functions that no permanent tooth can make up for if they're lost too soon.

What you do at home every night matters just as much as what we do at the clinic. These four habits make all the difference.

The first brush before the first tooth

From 4–5 months, before any tooth appears, clean the gums with damp gauze or a silicone finger brush. Once the first tooth comes through, move to a small toothbrush with a rice-grain-sized smear of fluoride toothpaste. The habit is set from the very start.

Fluoride: the right amount for each age

Up to age 3, use a rice-grain-sized amount of toothpaste. From age 3 to 6, a pea-sized amount. From age 6 onwards, a normal amount. Fluoride is the most effective ally for strengthening enamel, but the dose needs to be right. We'll tell you exactly which toothpaste to use and how much at your appointment.

Sugar: when and how, not just how much

The bacteria that cause decay feed on sugar and produce acid for around 20 minutes after each intake. What causes the most damage isn't the total amount of sugar, but how often it's eaten. Five biscuit bites through the day are far worse than eating all the biscuits in one go. Limit sweet snacking between meals.

No bottle or breastfeeding at night after 12 months

Falling asleep with milk or juice in the mouth is the most common cause of rampant decay in babies. Saliva flow drops at night, the sugars in the liquid coat the teeth for hours, and bacteria act unchecked. If your child needs something to fall asleep, offer water. From their first birthday, no more night-time bottles.

The official recommendation is to take your child to the dentist when their first tooth appears, or before their first birthday, whichever comes first. This first visit isn't a treatment: it's a relaxed check-up where the paediatric dentist reviews how the teeth are coming through, advises parents on hygiene and diet, and builds a positive relationship with the child before any problem arises.
It's more common than it seems, and we have plenty of experience handling it. We use the Tell-Show-Do technique: we explain, we show, and we do, always at a pace the child can cope with. On the first visit we often don't touch anything at all: we just play, look and build trust. For severe anxiety, we offer conscious sedation with nitrous oxide, which relaxes the child safely without the need for general anaesthesia.
It doesn't hurt at all. Fissure sealants are one of the simplest treatments we do: no anaesthesia is needed, there's no instrument that causes an unpleasant sensation, and the whole process takes only a few minutes per tooth. We simply clean the tooth, apply a mild acid for a few seconds so the sealant bonds well, apply it and harden it with a special light. The child can eat as soon as they leave the appointment.
It depends on the case. The first orthodontic assessment is recommended at around age 6–7, when the first permanent teeth start to come through. If we spot jaw development problems, crossbite or lack of space, we can start interceptive orthodontics with orthopaedic appliances between ages 7 and 10 to make the most of the child's growth. Active orthodontic treatment with braces or aligners usually starts between ages 10 and 14, once the permanent teeth are in place.

Always. Decay in baby teeth needs treating even though the child will eventually lose them. A baby tooth has two critical functions: chewing properly for years and holding the space for the permanent tooth growing just beneath it. Untreated decay can infect that developing tooth and affect the permanent one. What's more, an early extraction due to decay can cause crowding later on.
The first permanent teeth usually appear at around age 6: the first permanent molar (the 'six-year molar') and the lower central incisors. The full replacement process continues until around age 12–13. At age 6–7 we recommend the first orthodontic assessment to catch spacing or bite problems early.
